Expressive or receptive aphasia can occur after stroke and impairs one's ability to express or understand language. Reading and writing can be effected as well. Expressive language disorders cause difficulty in articulating or saying words, but there is often good comprehension and understanding of spoken language. Goal setting should be a dynamic process that is reviewed across the continuum of care in order to reflect the client/family context, wishes and language recovery. Therapists should explain the goal setting process to the person with aphasia and their family in an accessible way.

Expressive aphasia, also known as Broca's aphasia, is a type of aphasia characterized by partial loss of the ability to produce language (spoken, manual, or written), although comprehension generally remains intact. A person with expressive aphasia will exhibit effortful speech. This type of aphasia is also known as non-fluent or expressive aphasia. Patients have difficulty producing grammatical sentences and their speech is limited mainly to short utterances of less than four words. Producing the right sounds or finding the right words is often a laborious process. Regardless of the stage of intervention (early post-onset through chronic stages), the overriding goal of all aphasia therapy is to improve communication when and where a person wants or needs to communicate.
